The Housing Based Case Management program is accessible to youth ages 16 to 24. Housing Based Case Managers (HBCM’s) provide individualized supports in order to assist homeless and/or at-risk youth in obtaining stable, safe, affordable, long-term housing. HBCM’s will also refer clients to other resources that support housing retention. Referrals would include, peer support, education, employment, mental health, substance abuse and recreation. HBCM’s work with clients on an ongoing basis to assess and support clients in gaining knowledge, tools and skills to live independently.

To gain access to this program youth are required to complete a “TAY-VI SPDAT” assessment. This assessment is then sent to the City of Ottawa Housing First Data Coordinator who will match the youth to a Housing Based Case Manager based on youth needs and priorities.  If you or someone you know would like more information or assistance on gaining access to this program please call: 613-230-4663 and ask for a housing staff.
